My Tenant signed N11, N12, but not vacating, please help

Hello Landlords,
Hope you are enjoying this Fall season of 2024

I have rental townhouse in Kanata, Ottawa, ON. My tenant is senior person (68+ years old) staying with his dog, was paying rent via his government and private pension that he receives
I want my tenant to vacate, here is summary of what happened

July-August:-
My tenant send me email at end of July 2024, saying his rent for August might be delayed as he has been scammed, he has no money

August:-
Tenant could bot get his money back and did not paid his August rent, I gave him for N12 (informing my adult daughter wants to move in this townhouse), he agrees to leave and also signs N11 form, agreeing to vacate end of Sept 2024
By the way my house, backyard, front yard is mess, dirty not cleaned, dog hair lying everywhere

September:-
Tenant did not paid rent for September (August rent is also not paid), telling me there is some issue with his bank account, I adjusted his rent deposit money towards rent for August
Tenant's brother lends him money and Tenant pays rent for September

I still want tenant to vacate at the end of September
Tenant contacted Housing Case Worker from "Housing Help" located at 1335 Carling Ave

I think he is getting help from, maybe a legal advice from "Housing Help" .... and now his language to respond me changes
Telling me "He has no where to go, New places not accepting his dog, He does not want to leave Kanata area, he does not have money"

I give my tenant extension to stay until end of October, provided he pays me rent in full for October

October:-
Tenant says he does not have money to pay me , he can pay 1/4 of the rent and looking for new places, but not finding new rental yet

I am really tired at this point as, this rental home is on Bank Mortgage and back in September there was mortgaged renewal and interest rate almost doubled

PLEASE GIVE ME GUIDANCE
1. "Hosing Help" NGO company contacted me to send them the N11 and N12 forms that my tenant signed
Should I be worried that my tenant is seeking help from "Housing Help" ?
Should I send N11, N12 to should "Housing Help"?

2. I think I should file a case in tenant board as I have N12 and N11 signed by Tenant?

3. Do you know any lawyer whom I can consult about this

4. Any other suggestions?

Any guidance suggestion will be a great help
